ECFMG Certification Requirements
The applicant must first establish a MyIntealth account and submit an Application for ECFMG Certification. The medical diploma and final medical-school transcript must then undergo primary-source verification.
This verification is completed directly with the issuing institution. A translation submitted by the applicant does not replace confirmation from the medical school.
The applicant must also pass USMLE Step 1 and USMLE Step 2 CK. Beginning in January 2026, the Federation of State Medical Boards provides USMLE Step services for international medical students and graduates.
Applicants must additionally satisfy the clinical and communication skills requirement through an applicable ECFMG Pathway. The Pathways include a satisfactory performance on OET Medicine, along with any other requirements associated with the selected pathway.

ECFMG, EPIC and MyIntealth
MyIntealth is the platform used to manage the ECFMG Certification process and related services. Applicants should follow the current instructions provided for their certification year.
EPIC is a separate Intealth service used to verify and transmit medical credentials to organizations that request EPIC reports. Opening an EPIC portfolio is not automatically the first step for every applicant pursuing ECFMG certification.
Confusing these services may lead to unnecessary fees or duplicate requests. Applicants should verify which platform is required for the exact purpose before submitting documents.
ECFMG Certification and the Residency Match
International medical graduates must satisfy the ECFMG requirements established for the relevant National Resident Matching Program cycle. The NRMP verifies the required examination and pathway status by its Rank Order List Certification Deadline.
Applicants do not necessarily need to hold the final physical certificate by that deadline in every situation. However, they must satisfy the ECFMG verification requirements applicable to Match participation.
A valid ECFMG certificate is required before beginning an ACGME-accredited residency or fellowship. Certificates obtained through the Pathways may be subject to expiration rules unless they later become valid indefinitely under ECFMG policy.

J-1 and H-1B Options for Medical Training
Many residency programs use the J-1 physician category sponsored through Intealth’s Exchange Visitor Sponsorship Program. Applicants generally need a contract or official offer and a Statement of Need from the Ministry of Health of their country of citizenship or most recent legal permanent residence.
J-1 physicians are generally subject to the two-year home-country physical presence requirement. Remaining in the United States after training may require a qualifying waiver or compliance with that requirement.
Some residency programs sponsor H-1B status, but availability varies by institution. Clinical H-1B cases commonly involve ECFMG certification, USMLE Step 3 and the appropriate state training license or permit.
There is no separate “ECFMG VisaScreen Certificate” for physicians. VisaScreen is associated with credentialing requirements for certain other healthcare professions and should not be presented as an additional ECFMG document for medical residency.

Common Mistakes That Delay Certification
One common mistake is beginning the process without verifying the medical school’s World Directory listing and Sponsor Note. Without the correct ECFMG eligibility, later examination or document plans may not proceed as expected.
Another error is assuming that uploading a diploma completes credential verification. The medical school must respond to the verification request through the procedure established by Intealth.
Candidates may also follow outdated Pathway instructions. ECFMG requirements, application periods and certificate-validity rules can change between certification cycles.
The immigration and credentialing timelines should also be treated separately. Passing the required examinations does not create work authorization, and obtaining a visa does not establish eligibility to practice medicine.

The total timeline depends on examination preparation, test availability, Pathway deadlines and the medical school’s response to credential-verification requests. For that reason, a single standard completion period cannot be guaranteed.
As of April 2026, published fees include US$110 for establishing a MyIntealth account, US$580 for the Application for ECFMG Certification and separate charges for verifying the diploma and transcript. Examination and Pathway-related fees must be added to the overall budget.
